Implements WI-2: add YAML front-matter per docs/plans/README.md schema; normalize H1 titles; remove redundant inline metadata (Author, Date, Revised, Issue, Branch, Status). Statuses: 441=implemented, 451=ready, 468=ready. No changelog/body/squad-cli changes. Co-authored-by: Copilot <223556219+Copilot@users.noreply.github.com>
